1. Project Background

OmniHealth operates a large network of primary care providers and behavioral health specialists. To improve patient care coordination, they needed a secure provider-to-provider consultation platform. This system would allow primary care doctors to quickly initiate video consultations with remote psychiatrists, securely share relevant patient clinical records, and collaborate on treatment plans in real-time.

2. The Challenge

OmniHealth had to overcome massive integration hurdles with multiple disparate Electronic Health Record (EHR) systems. Sharing protected health information (PHI) across systems securely meant implementing HL7/FHIR protocols. Additionally, they required low-latency, high-definition video streaming that could perform reliably on standard clinical networks without requiring software installations, while keeping session metadata fully encrypted.

3. Our Solution & Approach

We engineered a web-based, zero-install telehealth console using Next.js and WebRTC. The system implements a robust microservices backend written in Node.js and TypeScript, deployed on an AWS EKS (Kubernetes) cluster. For the video channels, we built a secure SFU (Selective Forwarding Unit) architecture using Mediasoup, ensuring high-quality, encrypted streams even under constrained network conditions. We integrated the platform with Epic and Cerner EHRs using the FHIR API, enabling automatic, secure synchronization of patient records and consult summaries.

4. The Results & Impact

The telehealth platform reduced the average wait time for patient psychiatric consultations from 14 days down to under 2 hours. Over 85% of primary care providers reported that the platform significantly improved their ability to manage complex behavioral health patients in-house. Security audits confirmed full compliance with HIPAA and SOC 2 Type II guidelines.

Being able to pull in a psychiatric specialist on the fly while a patient is in my office has saved lives. The integration is seamless and the video quality is outstanding.
